      What You'll Do<\/b>
      <\/p>

      • Manage a home health caseload throughout Bloomington and Eden Prairie<\/b>, with on -call and weekend visits within approximately 35 miles of your home.<\/b>
        <\/li>
      • Complete OASIS assessments and individualized plans of care.
        <\/li>
      • Deliver skilled nursing care and patient education.
        <\/li>
      • Coordinate care with physicians and interdisciplinary team members.
        <\/li>
      • Help patients achieve their goals while remaining safely at home.
        <\/li>
      • Maintain timely and accurate documentation.
        <\/li><\/ul>

        What We're Looking For<\/b>
        <\/p>

        • Active Minnesota Registered Nurse (RN)<\/b> license.
          <\/li>
        • 3+ years of RN experience<\/b> (home health experience preferred).
          <\/li>
        • Strong clinical assessment and critical thinking skills.
          <\/li>
        • Excellent communication and organizational abilities.
          <\/li>
        • Reliable transportation for local travel.
          <\/li><\/ul>
