Responsibilities:- Facility based: The RN will visit members at assisted living / independent living facilities to perform hands on assessments with the possibility of wound care and simple nursing tasks and at nursing homes to implement member’s plan of care
- Remote based: The RN will complete paperwork and assessments from home
- In close collaboration with assigned provider, the RN case manager develops, implements, and monitors care plan, and ensures continuity of care across all healthcare settings
- Ensures that interdisciplinary team (NP/PA, PCP, member/member’s representatives, and specialists) are involved in care planning process
- Utilizes benefit structure and community-based resources to meet member’s needs
- RN case manager is required to be on site in ISNP / IE-SNP partnered facilities
- RN BSN, Masters Degree preferred
- Minimum two years clinical work in long term care or community settings servicing geriatric population,managed care or home care experience preferred
- Working knowledge of Medicare and Medicaid regulations
- Prior experience in an interdisciplinary service delivery environment preferred.
