In this role, you will perform thorough assessments of patients' health, considering physical, emotional, and social aspects. Collaborate with a diverse team including doctors, therapists, and social workers to curate and oversee personalized care plans. Your responsibilities will encompass skilled nursing tasks like wound care, managing medications, delivering IV therapies, and facilitating education for patients and caregivers. As a support system, you will provide essential guidance and emotional backing to families. Leadership duties include supervising LPNs/LVNs and support teams, ensuring high standards of care. The position demands participation in on-call services ready for urgent situations. Efficient care coordination and accurate documentation to meet legal and agency guidelines are crucial. Monitoring patients' improvements and evolving their care regimens based on their progress is expected. Applicants must hold a nursing degree, be licensed in Oregon, and have at least one year of RN experience.
