Position Summary

The RN Case Manager is responsible for overseeing a specific caseload, ensuring timely assessment, planning, implementation, and evaluation of discharge plans and care transitions throughout the care continuum.

Location: Main Campus

Hours: Full-Time, 32 Hours

Job Responsibilities

Essential Qualifications:
1. Valid RN License in Massachusetts.
2. Minimum of 3 years of RN experience.
3. Current Basic Life Support (BLS) Certification.

Preferred Qualifications:

1. Bachelor of Science in Nursing (BSN).

Key Duties and Responsibilities: The following tasks outline the general nature of the role and are not exhaustive. Additional responsibilities may be assigned as needed.

  • Conducts interviews and gathers detailed information from patients and their families, aiding in the identification of specific physical, emotional, educational, and cultural needs.
  • Maintains thorough documentation of assessment results, discharge plans, and actions taken in accordance with departmental protocols.
  • Completes discharge assessments, ensuring all documentation aligns with established policies and procedures.
  • Identifies obstacles to the efficient and cost-effective delivery of care.
  • Determines expected outcomes tailored to each patient.
  • Collaborates with physicians, healthcare teams, and patients/families in care planning and decision-making during hospitalization and discharge preparation.
  • Implements care plans to achieve the identified outcomes for patients.
  • Utilizes community resources and systems to facilitate the implementation of care plans, including making referrals when necessary.
  • Provides education to patients and families, documenting this according to policy.
  • Coordinates activities related to financial and insurance matters, including providing clinical updates to insurance providers.
  • Manages insurance appeals concerning discharges, adhering to Medicare and Medicaid policies and procedures.
  • Contributes to achieving unit quality scores that meet or exceed established targets for nurse-sensitive quality indicators relevant to the unit.
