Job Summary
Join Aegis as a RN Case Manager where you will play a crucial role in delivering exceptional care to our patients. Your compassionate approach will ensure comprehensive physical, social, and psychological assessments, and your friendly and supportive demeanor will actively engage patients, families, and the rest of our Interdisciplinary TEAM in developing comprehensive care strategies and plans. With your attention to detail, you will efficiently coordinate patient care delivery, manage resources, and maintain meticulous patient records. Your positive collaboration with other TEAM members, patients, and their families and proactive initiation of referrals will create a holistic care experience. Desired Skills & Qualifications
Current Registered Nurse (RN) license in Nebraska
Hospice experience as a Registered Nurse required
Home Health experience as a Registered Nurse preferred
Ability to utilize full range of movement to transfer patients
Effective verbal & written communication and time management skills
Valid NE Nursing License
Valid Driver's License (clean record) and reliable transportation
Proof of current auto insurance
Current CPR Certification
Negative TB Test
