Job Title: RN Case Manager
Location: Fort Walton Beach, FL
Type: Full-Time | Permanent | Day Shift
Schedule: 8:00 AM – 4:30 PM | Rotating Weekends
Compensation:
• $34.25/hr (Minimum)
• $42.82/hr (Midpoint – ~10 years experience)
• $47.10/hr (Maximum – 15+ years experience)
Sign-On Bonus: $10,000 (2-year commitment)
Relocation Assistance: Available (case-by-case)
Position Overview:
Navitas Healthcare, LLC is seeking RN Case Manager for an exciting Travel or Local job in Fort Walton Beach, FL.
Key Responsibilities
• Guide patients and families through rehabilitation and care program orientation
• Educate patients and caregivers on treatment plans, insurance benefits, discharge criteria, and patient rights
• Develop and implement individualized patient-centered care plans
• Complete psychosocial assessments and discharge planning evaluations
• Coordinate family conferences and interdisciplinary treatment discussions
• Collaborate with physicians, nurses, and care teams
• Advocate for safe discharge planning and continuity of care
• Maintain accurate and timely clinical documentation
• Promote compassionate and high-quality patient care
Qualifications
• Active Registered Nurse (RN) license required
• Associate Degree or Nursing Certificate required
• Minimum 2 years acute care experience within last 3 years required
• Preferred backgrounds: ICU, CCU, Med-Surg, LTAC, or Case Management
• Strong communication, critical thinking, and patient advocacy skills required
