      What You'll Do<\/b>
      <\/p>

      • Float between home health territories throughout the Twin Cities based on patient needs.
        <\/li>
      • Complete skilled nursing visits, OASIS assessments, and individualized care plans.
        <\/li>
      • Coordinate care with physicians and interdisciplinary team members.
        <\/li>
      • Support local teams during census growth, vacations, and coverage needs.
        <\/li>
      • Deliver compassionate, one -on -one care that helps patients remain safely at home.
        <\/li>
      • Maintain timely and accurate clinical documentation.
        <\/li><\/ul>

        What We're Looking For<\/b>
        <\/p>

        • Active Minnesota Registered Nurse (RN)<\/b> license.
          <\/li>
        • Strong clinical judgment and adaptability.
          <\/li>
        • Comfortable working independently in a field -based environment.
          <\/li>
        • Home health experience preferred, but not required.
          <\/li>
        • Reliable transportation and willingness to travel throughout the metro.
