Job Title:Registered Nurse (RN) Case Manager – Home Health | St. Paul Territory

Location:St. Paul, MN and surrounding communities within approximately a 35-mile radius of your home

Schedule:Full-Time | Monday through Friday, 8:00 AM to 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation:$85,000 to $130,000 annually plus $6,000 in first-year bonuses

The Role You'll Play

As the primary nurse overseeing your patients, you'll become the clinical leader coordinating every aspect of their care.

You'll partner with physicians, therapists, caregivers, and family members to create individualized care plans while helping patients avoid unnecessary hospitalizations and maintain their independence.

Every visit is an opportunity to improve outcomes, answer questions, solve problems, and provide reassurance during recovery.

What You'll Be Doing

  • Manage an assigned home health caseload throughout the St. Paul territory
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Develop and update individualized care plans
  • Perform skilled nursing visits and comprehensive patient assessments
  • Educate patients and caregivers on medications, diagnoses, and treatment plans
  • Coordinate care with physicians and interdisciplinary healthcare teams
  • Complete documentation using Homecare Homebase (HCHB)
  • Monitor patient progress and adjust care plans as clinical needs change

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Three or more years of RN experience
  • Home health experience preferred, but experienced clinicians from other settings are encouraged to apply
  • Strong assessment, organization, and critical thinking skills
  • Comfortable working independently while managing a daily schedule
  • Reliable transportation for travel throughout the St. Paul service area
