Qualifications and Skills
Active, unrestricted Registered Nurse (RN) license (Kentucky or Compact)
Associate’s or Bachelor’s degree in Nursing (or related field)
Home Health RN experience required
Strong OASIS documentation experience required
Homecare Homebase (HCHB) experience required
Ability to analyze complex clinical information and make sound care management decisions
Case Management certification (CCM or equivalent) preferred
Comfortable covering a multi-county territory (coverage discussed; up to 16 counties)
16 counties but will not cover all counties will discuss
Responsibilities (RN Case Manager):
Perform comprehensive assessments for high-risk patients/members in the home health environment
Collaborate with primary care providers and interdisciplinary teams to implement individualized care plans
Coordinate care transitions with physicians, social workers, discharge planners, and other clinical partners
Identify and address barriers to care plan compliance (e.g., transportation, housing, non-medical support needs)
Reassess appropriateness of level of care and support cost-effective, clinically appropriate interventions
Engage specialty resources as needed to drive optimal outcomes and resolution
Maintain detailed documentation of clinical, functional, and outcome measures (including fiscal tracking when applicable)
Promote health education, illness prevention, and early intervention to reduce avoidable adverse events
Protect patient privacy and confidentiality; act as a patient advocate in all interactions
RN Case Manager (Home Health)
Location: Lexington, KY (Onsite/Field-Based)
Schedule: Monday–Friday, 8:00 AM–5:00 PM + on-call (1 weekend/month + 1 weekday/week)
Pay: $56.14/hr
