Position Summary
V-Tech Solutions isseekingaRegistered Nurse(RN)CareManager toprovidecomprehensive care coordination services for children and young adults with special healthcare needs.The RN Care Manager assesses member needs, develops and monitors individualizedcare plans, coordinates services, and collaborates with families, providers, and community partners toimprove health outcomes and quality of life.
KeyResponsibilities:
- Manageacaseloadofmemberswithhigh-complexitymedicalandbehavioralhealthneeds
- Conducthealthassessments,outreach,andcommunityorhomevisits
- Develop,implement,andmonitorindividualizedcareplansbasedonacuity
- Coordinatecarewithphysicians,specialists,caregivers,andmultidisciplinaryteams
- Supporthospitaldischargeplanningandtransitionsofcare
- Educatemembersandcaregiversonconditions,medications,andself-management. Identifygaps incareandaddress over- orunder-utilizationofservices
- Documentcaremanagementactivitiesaccurately andtimely per policy
Education&Experience
- Associate'sDegreeinNursingrequired (Bachelor'sDegreepreferred)
- Minimum 2yearsofexperience incaremanagement, discharge planning, homehealth,ormanaged care
- Atleast1year ofexperienceinapublicorcommunityhealthsetting
Licensure&Certification:
- ActiveRegisteredNurse(RN)licenseinWashington,DC(requireduponhire)
- CertifiedCaseManager (CCM)preferred
