You’ll work with a collaborative, mission-driven team supporting older adults and medically complex patients across multiple care settings — from clinics to homes — ensuring continuity, dignity, and quality of life.
Deliver high-quality, compassionate nursing care across clinic, home, and community settings
Conduct comprehensive patient assessments and develop individualized care plans
Coordinate care across providers, specialists, and support services
Manage chronic and acute conditions, ensuring seamless transitions of care
Perform medication reconciliation and monitor treatment adherence
Educate patients and families on health management and safety
Participate in a 24/7 on-call rotation for patient support
Collaborate with CNAs and care teams to ensure optimal patient outcomes
Active RN license (required)
1+ year of nursing experience
Strong communication and care coordination skills
Passion for working with elderly or medically complex populations
Ability to work independently while thriving in a team environment
Reliable transportation (travel required for home visits)
Experience in home care, managed care, or long-term care
Familiarity with EMR systems
BSN (preferred)
Strong understanding of patient-centered care models
