Position Purpose:

The Behavioral Health Nurse provides assessments, support and care coordination across the behavioral health care continuum for patients struggling with mental illness as well as individuals and families seeking resources or information about treatment options. The Behavioral Health Nurse is an integral member of Behavioral Health Service Line and works collaboratively with the treatment team members and providers in the various inpatient and outpatient programs offered at Mercy. The Behavioral Health Nurse works closely with community partners to ensure successful transition of care and promotes education to the public on Behavioral Health-related topics through various forums. The Behavioral Health Nurse is responsible for carrying out duties as a health coach as well as utilizing skills as a psychiatric nurse in various care areas when needed, including but not limited to the Intensive Outpatient Program and Psych Triage in the Emergency Department.

What you will Do:

  • Assesses and evaluates individuals' physical, psychosocial, environmental, spiritual, social and education needs.

  • Works collaboratively with the Behavioral Health inpatient and outpatient teams to achieve desired patient care outcomes and ensure a smooth transition of care

  • Provides, facilitates, and/or implements learning opportunities to meet the educational needs of patients, families or members of the community.

  • When required, documents patient information in the electronic/paper medical record in an accurate, timely manner

  • Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.

  • Effectively communicates pertinent patient changes/conditions to colleagues and to multidisciplinary caregivers.

Hours/Schedule:

Full Time 36hrs a week,

night 12hr shift 7pm - 7:30am, every third weekend, every other holiday

Minimum Qualifications:

  • Current RN Licensure in the state of Iowa, or qualified multistate licensure required.

  • Bachelor of Science in Nursing preferred

  • Previous clinical experience in a psychiatric treatment setting, acute care preferred

  • Ability to be non-judgmental regarding living and health related decisions.
