POSITION TITLE: Residential Service Coordinator

LOCATION: Nashville, TN

STATUS: Full Time, Salaried, Exempt

REPORTS TO: Director of Developmental Disability Services

JOB SUMMARY AND QUALIFICATIONS:

The Residential Service Coordinator is responsible for the overall services for those individuals receiving Residential Habilitation or Supported Living services from Volunteers of America. The implementation of the Individual Support Plans and the attainment of individual's visions within their home and community environments is essential. This person will be responsible for case management and referral to resources in the community as well as direct supervision of house managers in each residential and supported living setting.

WHAT YOU SHOULD HAVE FOR THIS ROLE:

This position requires a BA degree OR an equivalent number of years of progressively more responsible direct experience with the consumer population AND a current Tennessee Driver's License.

RESPONSIBILITIES:

  1. To ensure compliance with DMH/DD Quality Enhancement guidelines for residential supports and services.

  2. To ensure cleanliness and safety of persons served and their homes.

  3. To ensure the implementation of Individual Support Plans, and ensure that monthly reviews encompass pertinent information that is exchanged with the Medical and Day Service Coordinator as well as pertinent persons from individuals, families, guardians, Independent Support Coordinators, Regionals Office personnel, etc.

  4. To complete comprehensive monthly reviews in a timely manner.

  5. To monitor consumers' outcomes to ensure the agency actively strives towards reaching the person's vision.

  6. To monitor the consumer personal records to ensure information contained is in accordance with DMH/DD guidelines.

  7. To coordinate all residential employee training and to assist in tracking training to ensure best practice and DMH/DD compliance.

  8. To coordinate services with individuals served, guardians, families, Independent Support Coordinators, day services, etc.

  9. To participate in management meetings and supervisory planning sessions.

  10. To ensure that individuals served receive all possible benefits for the purpose of increasing their disposable income, i.e. food stamps, MR housing, level billing, discount utility programs, STRAP etc.

  11. To ensure the agency supervision and all internal quality assurance plans are implemented as they pertain to residential services and supports.

  12. To ensure that prudent financial residential spending occurs and all appropriate documentation regarding expenditures meets policy and procedure requirements.

  13. Any other responsibilities as assigned to meet the mission of the agency.

  14. Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.
