Position Description
The Case Manager is responsible for completing clients' reports and connecting the client to needed resources. 
 
Qualifications 
  1. A bachelor's degree, preferably in social work, psychology, or an equivalent discipline.
  2. A minimum of one year experience in a residential setting, preferably with troubled latency and adolescent clients.
  3. Pass a criminal background check (FBI, DOJ, and Child Abuse), TB test, physical exam, and drug test. 
  4. Have a valid driver's license and meet insurance requirements. 
  5. Current CPR and First Aid certificates. 
  6. The ability to author emails, letters, reports, and other documents professionally.  
  7. Proficient with Microsoft Office Suite or related software. 
  8. Organizational skills that include managing time efficiently and workload effectively.  
  9. Exceptional communication skills, both oral and written.
  10. Strong leadership and management abilities to effectively oversee staff and operations. 
  11. The ability to work collaboratively within a team environment and inspire a positive and inclusive work culture.
  12. The ability to work effectively under pressure, managing multiple tasks and responsibilities simultaneously. 
  13. The ability to work independently, be a self-starter, and be energetic.
  14. Must be able to work a flexible schedule that may include extended hours, evenings, weekends, and holidays.
  15. The personnel file shall contain documentation of the required education and experience, an age affidavit, and a confidential form.
 
Examples of Duties and Responsibilities
  1. Collaborates with the client, staff, CFT, and other supportive entities to meet the client's needs.
  2. Prepare the Quarterly, Discharge, and Needs and Services Plan reports as required, ensuring that all required signatures are included. 
  3. Responsible for ensuring that all reports are turned in on time and approved by the HOS. 
  4. I track report due dates and coordinate with the administration to ensure that the information required to complete reports is received in a timely manner. 
  5. Meet regularly with the clients and the treatment team to establish goals.  
  6. Assists with linking clients to needed services, including scheduling external appointments.
  7. Assist the HOS with quality assurance duties pertaining to mental health, including but not limited to audit preparation, chart evaluations, and file maintenance.
  8. Attend treatment team, case conferences, and CFT meetings.
  9. Adheres to the agency's safety protocols.
  10. Organize and facilitate resident council.  
  11. Transport clients to ILS activities.  
  12. Other duties as assigned.
