Position Summary:

The Residential Case Manager provides individualized case management and advocacy services to adults living in a residential facility for individuals with mental illness. This role ensures that each resident receives the supports necessary to maintain stability, pursue recovery goals, and work toward greater independence. The Case Manager works closely with clinical and residential staff to coordinate services, monitor progress, and connect residents with community-based resources.

Key Responsibilities:

  • Develop, implement, and regularly update individualized service plans (ISPs) in collaboration with residents, treatment teams, and support networks.
  • Serve as the primary point of contact for a caseload of residents, ensuring consistent support, advocacy, and documentation.
  • Coordinate referrals to mental health, medical, vocational, housing, and social services as needed.
  • Assist residents in developing independent living skills, including budgeting, medication management, self-care, and community navigation.
  • Conduct regular one-on-one meetings with residents to assess needs, monitor progress, and support personal goals.
  • Maintain accurate, timely, and confidential documentation of services, assessments, progress notes, and critical incidents.
  • Participate in multidisciplinary team meetings, clinical case reviews, and discharge planning processes.
  • Advocate for resident rights and promote self-determination and recovery-oriented practices.
  • Support crisis intervention efforts, providing de-escalation and coordination with emergency services as needed.
  • Help plan and support resident participation in structured daily activities, community outings, and therapeutic groups.
  • Maintain compliance with all state, federal, and organizational standards and protocols
  • All other duties as assigned.

Qualifications:

  • Bachelor's degree in social work, psychology, human services, or related field required.
  • Minimum of 1-2 years of experience working with individuals with mental illness in a residential or community setting preferred.
  • Knowledge of mental health systems, recovery models, and community resources.
  • Strong communication, documentation, and problem-solving skills.
  • Ability to work collaboratively across clinical, administrative, and residential teams.
  • Flexibility to adjust schedule based on resident needs or emergencies.
  • Valid driver's license and reliable transportation (if required by program).
  • Must pass background check and meet all regulatory clearance requirements.

Transportation Requirements:

  • Provide transportation for consumers as needed using agency or personal vehicles.
  • Maintain a safe driving record and valid driver's license with personal liability insurance.
  • Follow all agency transportation policies, including seatbelt usage, safe cellphone practices, and vehicle documentation.
  • Immediately report any vehicle issues, accidents, or violations to appropriate personnel.

Work Environment & Schedule:

  • This is a non-exempt, full-time position with a variable shift.
  • This position requires in-person work and is not eligible for remote or hybrid work arrangements.
  • Case Managers will provide services in community settings, including clients' homes, public spaces, and other locations as needed.
  • Must be able to travel within the service area on a regular basis and maintain a consistent in-person presence to meet consumer and program needs.

In-Service Training & Continued Education Requirements:

  • Certification: Must obtain Case Management Certification within 6 months of hire (agency support provided).
  • Attendance at all required in-service trainings as designated by the agency.
  • Ongoing professional development to maintain best practices and compliance with evolving standards of care.
  • Completion of agency-mandated onboarding and annual training modules.

Pay Range: $15/hour minimum and $19/hour maximum
