Toidentifythesocial,emotional,employment,education,andmedicallyrelatedneedsofall residents on an individual and group basis and provide services to meet those needs.

A. ServiceDevelopment,Linkage,andCoordination
• Identifies,distributesinformationabout,andlinksresidentstoexistingcommunityservices that help to improve housing stability, increase self-sufficiency, and build safe and supportive communities.
• Conductsassessmentstodetermineindividualandgroup-levelresidentgoals,interests, and needs.
• Createsanetwork,anddirectory,oflocalsocialserviceagenciestofacilitateresidents’ access to resources, programs, and services.
• Maintainsconfidentialitywithrespecttoinformationsharedbyresidentsandservices provided to residents. Professionalism is essential in this position.
• Workswithresidentstodevelopasenseoffellowshipandcommunityusing organization and facilitation skills. This may involve the development of resident associations, community-based newsletters, welcoming committees, orientation packages, support groups, etc.
• Assistsresidentsin securingand/orcreating social,community-oriented programming opportunities,whichmeettherecreational,health,educationalandspiritualneedsof the housing community.
• Raisesawarenessamonglocalserviceprovidersoftheneedsofresidentsofthehousing community and promotes partnerships that helps residents gain access to needed services.
• Refersforcasemanagementanyresidentexperiencingproblemsthatthreatenhealth, safety and continued independence, while supporting the resident in remaining at home.
• Providesfollow-uptoreferralstoensureappropriateservicedelivery.
• Helpsresidentsapplyforfairhearingswhenthereisreasontobelievetheresidentis eligible for a denied service.
• Helpsmarketthehousingpropertythroughnetworkingcontactsinthecommunity.
• Keepsappropriateresidentfilesthatdocumentconversationswithoraboutresidents, problems identified, referrals made on behalf of residents, residents' refusal or acceptance of services, and residents’ progress.
• Completesreportsregardingservicesprovided,residentoutcomes,andresident satisfaction, as required.
• Developsaworkingknowledgeofhousingrulesandissuessuchasreasonable accommodations, the lease and lease violations, etc.
• Assumesadditionalresponsibilitiesasrequested.
• Knowledge of community support services available to residents of family and/or senior housingdevelopmentsaswellastheeligibilityrequirementsforaccessingthoseservices.
• Experience working with older persons and/or families in a direct service capacity, especiallyintheareasofadvocacy,self-sufficiency,servicenegotiation andcommunity building.
• ExperienceusingMicrosoftOfficesuite(Word,Outlook,Excel,etc.),datamanagement tools (i.e., databases), and the internet.
• Abilitytoestablishtrustingrelationshipswithresidents;individually,andasagroup.
• Abilitytolistentoandassistresidentsindefiningtheirproblems;tohelpidentifypossible solutions and resources.
• Abilitytoseekinputfromresidents,toresearchpossibleservicevendors,toevaluate services delivered and make adjustments, if needed.
• Abilitytocounseloradvocateinnon-judgmentalwaystoempowerresidentstomake personal decisions and enhance their self-esteem and independence.
• Ability to develop community-oriented activities that build bridges between people, promoteactiveinterestinthelifeoftheproperty,andgenerateresident self-sufficiency.
• Abilitytoeffectivelyplan,organize,develop,implementandinterpretprograms,goals, objectives, policies and procedures.
