JOB SUMMARY AND PURPOSE
The Resident Care Manager I advocates for residents and provides intensive care and intensive case management services to eligible clients, meeting participants at least weekly. The Resident Care Manager is the primary point of contact to coordinate services and care for assigned clients. Each Resident Care Manager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available
DUTIES AND RESPONSIBILITIES
Conducts comprehensive screenings and assessments with individuals (or families) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate
Assess barriers facing the clients and develop a case plan.
Coordinate individualized planning with clients to meet short- and long-term needs.
Provide service linkages and support systems to ensure identified needs are met.
Coordinates case conferences with residents, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.
Acts as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitates support groups and client participation as appropriate.
The Resident Care Manager I shall coordinate the participants care in collaboration with the participant, and all members of the participants care team (e.g., primary care physician, mental health service provider, substance use disorder service provider, ECM providers, housing navigators, street outreach case managers, family members, etc.) and support the day-to-day operations of the Community by performing the following duties:
Works with Community Property Management to provide service linkages and support systems to residents to assure individual needs are met
Maintains daily contact to identify new and ongoing service needs
Coordinates access to supportive services including, but not limited to, health services, medical appointments, and crisis intervention, as needed
