Position Summary
The Resident Care Coordinator is responsible for assisting the Director of Resident Care in all areas necessary to promote optimal care for residents and the efficient operation of the Assisted Living Department and/or Memory Care unit. This position coordinates all care services provided to residents by caregivers and medication aides in cooperation with the Director of Resident Care and plays a key role in staff development, training, and day-to-day clinical support.
Responsibilities
- Ensure the provision of quality services to residents and ensure that all Federal and State policies governing resident care are closely monitored and followed.
- Under the direction of the Director of Resident Care, assist with interviewing, hiring, evaluating, motivating, scheduling, supervising, and monitoring the performance of department staff in accordance with company policies.
- Support recruitment and selection efforts with a focus on qualified candidates who have an interest in caring for individuals with dementia.
- Make recommendations to the Director of Resident Care regarding staff evaluations, merit increases, disciplinary actions, and terminations.
- Coordinate departmental schedules and help ensure adequate staffing levels in accordance with company standards, budgets, policies, procedures, and state regulations.
- Oversee continuing education and staff development programs for caregivers and medication aides in compliance with Federal and State licensing requirements.
- Conduct or participate in new hire orientation to ensure employees understand departmental goals, policies, and procedures.
- Conduct or coordinate training to ensure competent medication administration, emergency preparedness, documentation, resident record maintenance, and acceptable treatment practices. Ensure all Assisted Living and/or Memory Care staff complete all required state training.
- Monitor and assist with personal care services and medication administration, ensuring all required charting, ADL documentation, and medication records are completed accurately.
- Arrange coverage for department responsibilities during staff breaks, meals, or absences through delegation or direct support as needed.
- Assist with resident assessments, including pre-admission, periodic, and change-of-condition assessments, in accordance with current regulations and company policies.
- Observe residents and communicate changes in physical or emotional condition to the Director of Resident Care and other appropriate team members. Participate in weekly service plan meetings as requested.
- Complete all required data entry into ECP to maintain accurate resident care information. Ensure Resident Care Profiles, assignment sheets, and all required documentation are completed in a timely manner.
- Assist in coordinating nursing services with other resident services to ensure continuity of care.
- Serve as a resource for residents and their families by coordinating with physicians, pharmacies, and other healthcare providers regarding care needs and medication orders.
- Coordinate prescription orders with physicians' offices and pharmacies as needed and assist with supervising the storage and distribution of medications.
- Assist the Director of Resident Care with monitoring infection control practices and ensuring staff compliance with safety programs.
- Conduct community tours and promote the community to prospective residents and families.
- Support administration with referral development and maintaining high occupancy.
- Participate in state surveys and inspections as directed.
- Help maintain a safe and secure environment for staff, residents, and guests, including coordinating safety accommodations for residents with special needs.
- Participate in fire and emergency safety procedures when needed.
- Maintain inventory of department supplies as directed.
- Provide the Director of Resident Care with information regarding resident care needs and the department's ability to meet those needs.
- Participate in daily "Stand-Up" meetings in the absence of the Director of Resident Care or as requested.
- Assist with department budget preparation and monitor departmental costs.
- Perform day-to-day clerical and administrative duties related to the position.
- Maintain an updated list of oxygen tank locations throughout the community and ensure the Concierge has access to the current list for emergencies.
- Promote a spirit of teamwork through positive and cooperative interactions with coworkers and other departments.
- Maintain a positive and professional image through actions and appearance.
- Perform other duties as assigned by the Director of Resident Care.
Qualifications
- High school diploma or equivalent required.
- Certifications as required by the state.
- One (1) year of experience providing residential care to the elderly or experience with specialized dementia care programs preferred.
- Six (6) months of supervisory experience in a healthcare-related field preferred.

Schedule & Availability
This is a full-time position. The regular work schedule will be either Tuesday–Saturday or Sunday–Thursday. Team members in this role are expected to be on call to assist with staffing needs outside of regularly scheduled hours, as necessary.
