Qualifications
Education
SLP: Master’s degree in Speech-Language Pathology from a CAA-accredited program
PT: Bachelor’s, Master’s, or Doctorate in Physical Therapy (based on graduation year requirements)
OT: Bachelor’s or Master’s in Occupational Therapy (based on graduation year requirements)
Experience
Minimum 3 years of experience as a PT, OT, or SLP (required)
Previous supervisory or leadership experience preferred
Experience in rehabilitation services or hospital-based therapy settings preferred
Licensure & Certification
Active or eligible PT/OT/SLP license in Indiana (required)
Must obtain licensure within 90 days if not currently active in state
SLPs must hold ASHA CCC certification
CPR certification required or must be obtained within 90 days
APTA/AOTA/ASHA membership preferred
Additional Requirements
Ability to lift up to 50 lbs; 100 lbs with assistance
Strong communication skills (verbal and written)
Valid driver’s license required if driving a company vehicle
Proof of insurance required if using personal vehicle for work-related duties
