About the Role
The Rehab Care Coordinator supports the physical occupational and speech therapy teams by identifying and coordinating patient medical equipment needs, managing patient care logistics, conducting documentation audits, facilitating community programming, and assisting the Rehab Services Director in achieving quality assurance and standards compliance. This role is essential in ensuring patients receive timely and effective care from scheduling and documentation to securing necessary resources and equipment.
Rehab Care Coordinator duties include:
• Carrying out follow-up treatments • Facilitating positioning and adaptive equipment • Assisting with other duties, including equipment and supply organization and maintenance, as assigned by the OTR or Rehab Services Director • Taking into consideration patient needs based on gender, age, cultural differences, and educational needs
Qualifications
Graduation from an AOTA approved Certified Occupational Therapy Assistant educational program and successful completion of the field work requirements.
Licensed to practice in the state of Montana.
BLS certification required.
Minimum of 2 years experience in rehab field strongly preferred.
Ability to communicate effectively and work well with other healthcare providers.
Ability to work independently and creatively.
Interest in continuing to learn and grow professionally.
Ability to hold in confidence all personal hospital and professional matters.
