A healthcare organization is seeking a Registered Nurse | UAS Assessor for a hybrid role supporting in-home assessments across New York State. This position combines field-based member visits with remote documentation to help determine care needs and support accurate service planning.
About the Opportunity:
Shift: Day shift
Schedule: Monday through Friday, with weekend availability as needed
Hours: Standard business hours, with operations ranging from 7:30 AM to 8:00 PM on weekdays and 10:00 AM to 6:30 PM on weekends
Setting: Hybrid
Responsibilities:
Conduct face-to-face in-home UAS-NY assessments within an assigned territory
Review medical history, medications, and functional status to support care planning
Apply UAS criteria to determine level of care and service needs
Complete assessments and related documentation using web-based systems
Communicate with members, caregivers, and interdisciplinary teams to support scheduling and care coordination
Qualifications:
Active New York State Registered Nurse license
Graduate of an accredited nursing program
Minimum of 2 years of home-based experience within the last 15 years
Experience with CHHA, LHCSA, or MLTC field work, or similar in-home assessment experience
Experience conducting assessments similar to UAS, such as OASIS
Experience providing care to homebound individuals or managing HCBS, hospice, or assisted living cases
Ability to conduct in-home assessments throughout an assigned New York region
Strong understanding of UAS-NY assessment criteria
Proficiency with web-based applications, Microsoft Outlook, Microsoft Word, Zoom, and virtual assessment tools
Excellent organizational, interpersonal, written, and verbal communication skills
Desired Skills:
Recent UAS-NY experience, including Role 40 or 45
Experience working with geriatric populations and chronic disease management
Combination of RN experience with prior paraprofessional or LPN home care experience
Experience working with diverse and multi-generational populations
Bilingual fluency in Spanish, Russian, Haitian Creole, or Chinese
