Job Description Summary

‎ 

Position Summary
The Registered Nurse (RN) at CareMore Health provides clinical support and care coordination for patients within CareMore’s value-based care model. Working as part of an interdisciplinary care team, the RN supports the delivery of high-quality, patient-centered care by assisting with clinical assessments, patient education, chronic disease management, and care coordination activities.
This role focuses on supporting patients with complex medical needs, improving health outcomes, and ensuring patients receive appropriate and timely care across the healthcare continuum. The RN collaborates closely with physicians, advanced practice providers, care managers, and other clinical staff to enhance patient experience and improve quality outcomes.

Key Responsibilities

Clinical Care & Patient Support

  • Perform clinical assessments including vital signs, patient history review, and evaluation of patient symptoms.
  • Assist providers with patient examinations, procedures, and treatments as needed.
  • Administer medications, injections, immunizations, and treatments in accordance with physician orders and clinical protocols.
  • Monitor patient conditions and report significant findings or changes in health status to the care team.
  • Support management of patients with chronic conditions including diabetes, heart disease, COPD, and hypertension.

Care Coordination & Patient Education

  • Provide patient education regarding diagnoses, treatment plans, medications, and preventive health measures.
  • Support patients and caregivers in understanding disease management strategies and follow-up care instructions.
  • Coordinate patient care activities including scheduling tests, referrals, and follow-up appointments.
  • Assist with patient outreach and follow-up to ensure adherence to care plans and preventive services.

Value-Based Care & Population Health

  • Support CareMore’s value-based care model by assisting with management of high-risk and chronically ill patients.
  • Participate in efforts to close care gaps and improve quality metrics.
  • Identify patients who may benefit from additional care management support or intervention.
  • Collaborate with care managers and providers to support proactive patient management.

Integrated Team-Based Care

  • Work collaboratively with physicians, nurse practitioners, physician assistants, care managers, medical assistants, and other clinical staff.
  • Participate in interdisciplinary care planning and case discussions when appropriate.
  • Facilitate communication between patients, providers, and care team members.

Documentation, Compliance & Quality

  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Ensure compliance with clinical protocols, organizational policies, and regulatory requirements.
  • Maintain strict confidentiality of patient information in accordance with HIPAA and other applicable regulations.
  • Participate in quality improvement initiatives and patient safety programs.

Qualifications (Required)

  • Current, unrestricted Registered Nurse (RN) license in the applicable state(s)
  • Graduate of an accredited school of nursing
  • Minimum 2 years of clinical nursing experience preferred
  • Experience in primary care, ambulatory care, care coordination, or chronic disease management preferred
  • Experience working with complex or Medicare Advantage patient populations preferred
  • Current BLS Certification
  • Satisfactory completion of required health screenings (including Tuberculosis testing where applicable)
  • Hep B vaccinations (all 3, titer or signed declination)
