How you'll make an impact in this role:Guide the recovery journey by performing comprehensive bio-psychosocial assessments that capture the full picture of a patient’s needs and strengths to inform a personalized care strategy.
Lead therapeutic interventions by conducting or co-leading group therapy sessions, providing patients with the essential coping tools and community support needed for healing.
Shape personalized treatment goals by actively participating in team conferences to refine interventions that address the specific age-related and clinical needs of each patient.
Empower patient progress by evaluating real-time responses to care and adjusting nursing plans, ensuring every intervention is purposeful and responsive to the patient’s evolving status.
