A Brief Overview

Responsible for the planning and development of the Case Management program in conjunction with the leader. Responsible for the implementation and maintenance of the Case Management Program. Provides leadership and direction for the Case Managers. Provides statistical information to the organization on the Case Management program. Responsible for hospital wide compliance to Medicare/Medicaid and commercial payor issues.

What you will do

  • Examines current care practices in collaboration with leadership and various service areas to develop strategies for improvements in patient care delivery.
  • Analyzes data from internal and external sources and collaborate with Quality Improvement Specialists to identify target diagnosis (DRGs) or service lines.
  • Monitors Case Management programs and provide monthly reports to Case Managers, Medical staff, QI Committees, and leadership.
  • Provides education to hospital staff on health care reform, Case Management, and identified target populations.
  • Maintains a working knowledge of Medicare/Medicaid regulations and serves as a resource to hospital staff on compliance to Medicare/Medicaid issues.
  • Prepares outcomes reports related to Relationship Based Care and assures that staff are appraised of progress towards goals.
  • Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems.

Required Education, License/Certification, or Work Experience:

  • Bachelor''s
  • Registered Nurse (RN) - Board of Nursing An active license in the state of practice Upon Hire
  • Minimum of 3 years in case management field.

Preferred Education, License/Certification, or Work Experience:

  • Master''s in nursing
  • Certified Case Manager (CCM) - Commission for Case Manager Certification (CCMC) Upon Hire
