Registered Nurse (RN) – Start of Care - SOC/Visits Home Care – Boynton Beach / Delray Beach

Matrix Home Care is seeking an experienced Registered Nurse (RN) to provide skilled nursing visits, including Start of Care (SOC), routine visits, follow -up visits, patient assessments, and care coordination throughout Boynton Beach, Delray Beach, and surrounding South Palm Beach County areas.<\/span>
<\/p>

This is a field -based home care position ideal for an RN who is comfortable working independently in patients’ homes, providing compassionate skilled nursing care, and completing accurate and timely clinical documentation.<\/span>

Key Responsibilities<\/span>
<\/p>

Provide skilled nursing care to patients in their homes.<\/span>
<\/p>

Complete Start of Care (SOC), routine visits, follow -up visits, and nursing assessments as assigned.<\/span>
<\/p>

Monitor patient condition and report any changes, concerns, or updates to the clinical team.<\/span>
<\/p>

Provide patient and caregiver education based on the plan of care.<\/span>
<\/p>

Follow physician orders, the established plan of care, and agency policies.<\/span>
<\/p>

Communicate timely updates regarding patient status, care needs, and visit outcomes.<\/span>
<\/p>

Complete all required clinical documentation accurately and timely.<\/span>
<\/p>

Maintain professionalism, reliability, and compassionate patient care while working independently in the field.<\/span>
