*$80000 - $90000 annual salary, depending on experience.
Make a meaningful impact every day. As a Hospice RN, you'll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life's most tender moments. You will:
Complete comprehensive assessments of patients and families to determine hospice needs
Develop and implement individualized care plans, revising as needed
Provide skilled nursing care, administer medications, and manage symptoms
Educate patients and families on care techniques, medications, and comfort measures
Maintain accurate clinical documentation and communicate changes to physicians
Participate in interdisciplinary team meetings and quality improvement initiatives
Provide emotional support and counseling to patients and families
Supervise hospice aides and delegate responsibilities appropriately

Requirements
Currently licensed as a Registered Nurse (RN) in the State of Arizona
Minimum of 2 years of nursing experience, hospice or home health experience preferred
Homecare Homebase (HCHB) experience preferred, not required
Valid driver's license and reliable, insured vehicle
Strong clinical skills and a passion for patient-centered care
