Scarlet Home Health Care is seeking a full-time RN Case Manager to join our dynamic team! The ideal candidate will have a heart for working with home-bound patients, a passion for making a difference in their well-being, and the clinical expertise to manage complex care needs in a home setting. The area of coverage for this position is Grove City and the surrounding area. What You'll Do: - Collaborate with physicians to address medication needs and make necessary adjustments to care plans.
- Lead and supervise other healthcare disciplines, including LPNs, aides, and therapy staff, to ensure that each patient receives the highest standard of care.
- Complete OASIS, recertifications, discharge documents, and other necessary documentation within the EMR system.
- Actively monitor and assess changes in patient conditions to help prevent re-hospitalization, ensuring proactive interventions.
- Provide education and counseling to patients' families about care processes, disease management, and injury recovery.
- Attend orientation sessions, team meetings, and Continuing Education Units (CEUs) to maintain professional growth.
