Essential Functions

  • Responsible for the daily management of an assigned patient caseload, ensuring timely visits and documentation to meet patient needs as prescribed in the established plan of care and MD orders.
  • Provides comprehensive, individualized nursing care in the patients home, supporting continuity of care and improved outcomes.
  • Collects and analyzes data from in-home assessments, telephonic contacts, and EMR information to identify patient needs, risks, and available resources.
  • Identifies patients at high risk for re-hospitalization, falls, or high utilization of resources and initiates appropriate interventions, including case conferencing.
  • Prioritizes multiple patient issues based on clinical urgency, patient and staff safety, and timely delivery of services.
  • Identifies barriers to care (language, literacy, psychological, etc.) and collaborates with leadership or appropriate resources to resolve issues effectively.
  • Develops, communicates, and oversees individualized plans of care that support quality outcomes and patient satisfaction.
  • Coordinates care across the interdisciplinary team, including supervision of LPNs and HHAs, to promote safe and effective patient-centered outcomes.
  • Performs ordered treatments, therapies, and procedures in accordance with State Nurse Practice Act, Home Health Conditions of Participation, JCAHO, and agency policies.
  • Evaluates patient progress toward established goals and adjusts care plans as needed.

Physical Requirements

  • Ability to walk, stand, sit, lift, reach, stoop, bend, push, and pull.
  • Must be able to lift and support up to 35 pounds for patient care, equipment, and supplies.
  • Clear communication in English; intact senses of sight, hearing, smell, and touch.
  • Finger dexterity, critical thinking skills, concentration, and ability to respond quickly to changes in patient condition.
  • Physical Abilities Testing required.

Education, Experience, and Certifications

  • Graduate of an accredited School of Nursing (BSN preferred).
  • BLS required per policy guidelines.
  • Current RN license (or temporary license as applicable) in the state of practice; NLC requirements apply as appropriate.
  • Additional education, training, certifications, or experience may be required per department leadership.
  • Valid drivers license, reliable transportation, and satisfactory driving record required.

Job Profile Summary Serves a case management role, responsible for an assigned group of patients, typically geographically related and accountable for the oversight and overall management of the post-acute services in the home setting. Provides direct patient care and supervision of LPNs and HHAs in accordance with an established plan of care to address the patient needs, individual goals and physician ordered services. Responsible for care coordination with the assigned multi-disciplinary team and acts as the assigned patient's liaison with other related healthcare providers in the continuum of care. Ensures appropriate, qualified and timely patient care and documentation in order to meet patient needs and agency financial, operational, and quality goals
