Primary Duties:
Provide in-home skilled nursing services
Evaluate patient health status and report concerns appropriately
Coordinate care with physicians and interdisciplinary teams

Ensure all documentation meets regulatory standards
Qualifications:
Current RN license in Arizona
Reliable transportation for travel throughout the service area
Excellent organizational and interpersonal skills
Home health experience preferred but not required
