What You'll Be Doing

  • Provide skilled RN home health visits throughout the St. Paul territory
  • Perform comprehensive nursing assessments
  • Deliver wound care, medication management, injections, and disease-specific treatments
  • Educate patients and caregivers on medications, diagnoses, and recovery plans
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Monitor patient progress and communicate changes in condition
  • Complete accurate documentation using Homecare Homebase
  • Help patients remain safely at home while reducing avoidable hospitalizations

Who We're Looking For

  • Active Minnesota Registered Nurse (RN) license
  • Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
  • Strong clinical judgment and assessment skills
  • Excellent communication and patient education abilities
  • Self-motivated and comfortable working independently
  • Reliable transportation and willingness to travel throughout the St. Paul service area
