    What You'll Be Doing<\/span><\/b>
    <\/p>

    • Manage a <\/span>home health<\/span><\/b> patient caseload throughout the <\/span>St. Paul<\/span><\/b> service area<\/span>
      <\/li>
    • Complete <\/span>OASIS<\/span><\/b> assessments and develop individualized care plans<\/span>
      <\/li>
    • Perform skilled nursing visits and comprehensive clinical evaluations<\/span>
      <\/li>
    • Coordinate care with physicians, therapists, social workers, and other healthcare professionals<\/span>
      <\/li>
    • Provide medication management, disease education, and ongoing patient support<\/span>
      <\/li>
    • Monitor patient progress and adjust care plans as needed<\/span>
      <\/li>
    • Complete documentation using <\/span>Homecare Homebase (HCHB)<\/span><\/b>
      <\/li>
    • Help patients remain safely at home while reducing avoidable hospitalizations<\/span>
      <\/li><\/ul>

      Who We're Looking For<\/span><\/b>
      <\/p>

      • Active Minnesota Registered Nurse (RN) license<\/span><\/b>
        <\/li>
      • Three or more years<\/span><\/b> of RN experience<\/span>
        <\/li>
      • Home health experience preferred<\/span>
        <\/li>
      • Excellent clinical assessment and critical thinking skills<\/span>
        <\/li>
      • Strong communication and leadership abilities<\/span>
        <\/li>
      • Comfortable managing an independent schedule<\/span>
        <\/li>
      • Reliable transportation for travel throughout the <\/span>St. Paul<\/span><\/b> territory<\/span>
