    Job Title:Registered Nurse (RN) Case Manager – Home Health | St. Paul Territory

    Company: Home Health Care, Inc.

    Location:St. Paul, MN and surrounding communities within approximately a 35-mile radius of your home

    Schedule:Full-Time | Monday through Friday, 8:00 AM to 5:00 PM, 1 weekend per month, 1 on-call shift per week

    Compensation:$85,000 to $130,000 annually plus $6,000 in first-year bonuses

    Lead the Plan of Care

    As a Case Manager, you're not simply responding to patient needs.

    You'll oversee each patient's episode of care, coordinate services across multiple disciplines, monitor progress, educate caregivers, and identify potential concerns before they become larger problems.

    Your clinical expertise becomes the foundation for safer recoveries and better patient outcomes.

    What You'll Be Doing

    • Manage a home health patient caseload throughout the St. Paul service area
    • Complete OASIS assessments and develop individualized care plans
    • Perform skilled nursing visits and comprehensive clinical evaluations
    • Coordinate care with physicians, therapists, social workers, and other healthcare professionals
    • Provide medication management, disease education, and ongoing patient support
    • Monitor patient progress and adjust care plans as needed
    • Complete documentation using Homecare Homebase (HCHB)
    • Help patients remain safely at home while reducing avoidable hospitalizations

    Who We're Looking For

    • Active Minnesota Registered Nurse (RN) license
    • Three or more years of RN experience
    • Home health experience preferred
    • Excellent clinical assessment and critical thinking skills
    • Strong communication and leadership abilities
    • Comfortable managing an independent schedule
    • Reliable transportation for travel throughout the St. Paul territory
