  What You'll Be Doing<\/span><\/b>
  <\/p>

  • Provide skilled <\/span>home health nursing visits<\/span><\/b> throughout multiple service areas<\/span>
    <\/li>
  • Perform comprehensive patient assessments<\/span>
    <\/li>
  • Deliver <\/span>wound care<\/span><\/b>, medication management, injections, and other skilled nursing services<\/span>
    <\/li>
  • Educate patients and caregivers on diagnoses and treatment plans<\/span>
    <\/li>
  • Collaborate with physicians, therapists, and interdisciplinary care teams<\/span>
    <\/li>
  • Float between territories to support census growth, vacations, and staffing needs<\/span>
    <\/li>
  • Complete timely documentation using <\/span>Homecare Homebase (HCHB)<\/span><\/b>
    <\/li>
  • Help ensure continuity of care for patients across the organization<\/span>
    <\/li><\/ul>

    Who We're Looking For<\/span><\/b>
    <\/p>

    • Active Minnesota Registered Nurse (RN) license<\/span><\/b>
      <\/li>
    • Experience in <\/span>home health<\/span><\/b>, hospital, rehabilitation, skilled nursing, or acute care<\/span>
      <\/li>
    • Strong clinical judgment and assessment skills<\/span>
      <\/li>
    • Comfortable adapting to different territories and patient populations<\/span>
      <\/li>
    • Excellent communication and organizational abilities<\/span>
      <\/li>
    • Reliable transportation and willingness to travel throughout the <\/span>Twin Cities Metro<\/span><\/b>
