        What You'll Do<\/b>
        <\/p>

        • Manage a home health caseload throughout Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.<\/b>
          <\/li>
        • Complete comprehensive OASIS assessments and individualized care plans.
          <\/li>
        • Deliver skilled nursing care and patient education.
          <\/li>
        • Collaborate closely with physicians and interdisciplinary team members.
          <\/li>
        • Help patients recover, remain independent, and avoid unnecessary hospitalizations.
          <\/li>
        • Maintain accurate clinical documentation.
          <\/li><\/ul>
