We have two great Full Time opportunities to join our team as an RN Case Manager:

1) Territory covers Warwick, Cranston, East Greenwich and surrounding areas

2) Territory covers Cranston, Johnston, North Providence, Lincoln and surrounding areas

*Travel outside of territory will be required based on agency needs.

The Registered Nurse (RN) Case Manager - Home Care contributes to our Mission of Caring by providing compassionate direct patient care to our clients in a one-on-one setting in their homes. The Registered Nurse (RN) Case Manager - Home Care conducts nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Registered Nurse (RN) Case Manager - Home Care for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Registered Nurse (RN) Case Manager - Home Care is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.

Qualifications for a Registered Nurse (RN) Case Manager - Home Care:

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience, home care experience preferred.
• CPR certification.
• Reliable transportation, current clean driver's license, automobile insurance.
