Responsibilities:

What You'll Do:

  • Provide services in accordance with the plan of care.
  • Make the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
  • Initiate the plan of care and necessary revisions.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  • Coordinate services.
  • Inform personnel of changes in the condition and needs of the patient.
  • Counsel the patient and family/significant others in meeting nursing and related needs.
  • Process orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
  • Determine the amount and type of nursing needed by each individual patient.
  • Supervise and teaches other nursing personnel.
  • Conduct patient care conferences on patients assigned to his/her care.
  • Participate in peer review and Quality Assessment and Performance Improvement as assigned.
  • Give total patient care as needed; Takes on-call duty nights, weekends and holidays, as assigned.
  • Complete and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
  • Appropriately utilize ICD-10 codes.
  • Perform other related duties as assigned.

Education and Experience:

  • Graduate of an accredited nursing school.
  • Minimum of one (1) year clinical nursing experience.
  • Georgia RN licensure.
  • Experience working with medically fragile clients, addressing medical, social and behavioral components of care.

 

Travel Requirements:

  • Regular travel on a weekly basis throughout an assigned territory

Pay Range: USD $82,000.00 - USD $82,000.00 /Yr.
