Registered Nurse (RN) – Case Manager
Location:Anchorage, Alaska
Shift: Day Shift – 5x8-Hour Shifts
Schedule: Variable Day Shifts (Typically 7:00 AM – 3:30 PM or 8:00 AM – 4:30 PM)
Contract Duration: 13 Weeks
Position Overview
We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join a leading acute care hospital in Anchorage, Alaska. This role is ideal for candidates with acute care and case management experience who thrive in a collaborative healthcare environment focused on care coordination, discharge planning, and patient advocacy.
The Case Manager RN will work closely with interdisciplinary teams to ensure patients receive appropriate, timely, and cost-effective care throughout their hospital stay and transition of care process.
Key Responsibilities
- Coordinate and manage patient care plans throughout hospitalization
- Collaborate with physicians, nursing staff, social workers, and interdisciplinary teams
- Facilitate discharge planning and care transition processes
- Review patient progress and ensure appropriate utilization of healthcare resources
- Communicate effectively with patients, families, and healthcare providers
- Maintain accurate and timely documentation within the electronic medical record system
- Advocate for patient needs while supporting quality and regulatory standards
- Float within scope of practice to support facility needs when required
Requirements
- Active Alaska RN License required
- Minimum 1 year of RN Case Management or related acute care experience required
- Acute Care experience required
- Epic EMR experience required
- Current BLS Certification required
- ACM Certification preferred
- Strong communication, organization, and care coordination skills
- Ability to work effectively in a fast-paced hospital environment
