    Description:

    Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. As a contributing member of a health care team, develops and provides a comprehensive plan of care for assigned patients. Eligible for shift (pm and night) and weekend differential. 

    Level 2 Requires 8-12 hrs. shifts in a 6-week schedule period including one (2) weekend shifts. This Level also requires one Winter and one Summer holiday

    Job responsibilities:

    Patient Care & Assessment - Conducts comprehensive initial and ongoing assessments of patient and family need for service. Clarifies and implements physician orders by administering medications and performing treatments and testing as prescribed by the physician, and documents accordingly. Recognizes symptoms and establishes priorities consistent with patient's condition. Documents ongoing assessments of patient's physical and emotional status in order to address patient needs appropriately.

    Plan of Care - Obtains information from patient and/or family to develop an appropriate patient plan of treatment. Formulates, implements and continually revises patient plan of care based on ongoing assessment of the patient and collaboration with the interdisciplinary team to promote effective patient care.

    Coordination of Care - Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Maintains strict confidentiality of patient, employee and department information to ensure patient rights. Participates in interdisciplinary team meetings. Adheres to department policies for submitting documentation. Ensures that arrangements for equipment and other necessary items and services are available. Assists with the daily oversight of other clinical staff (other nurse and non-nurse staff members) when directed by the manager to maximize workflow in the practice. Provides feedback to the manager when requested regarding work-related skills of non-licensed personnel. Serves as a mentor and preceptor for new staff and as a constant resource to non-licensed or non-nurse personnel.

    Quality Improvement – Utilizes time and resources effectively to promote quality and cost outcomes.

    Discharge Planning - Identifies discharge planning needs as part of the care plan development and properly notifies patient and family members.

    Education - Assesses patient and family in order to provide health education to promote their independence and safety with self-care. The nurse will assess current level of knowledge, readiness to learn, and learning ability in order to be effective. The nurse will document health education provided. The nurse will document learning outcomes in terms of the patient and/or family ability to verbalize understanding and provide return demonstration when appropriate.

    Licensure/Certifications:

    Required:

    Basic Life Support (CPR)

    General - Valid Illinois driver's license

    Nursing - Current Registered Nurse license State of Illinois

    BSN Pay Range: $34.48 - $59.92 per hour

    ADN Pay Range: $32.90 - $57.06 per hour

    Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
