Job Summary: The Registered Nurse (RN) is responsible for the overall management of nursing care for patients. This includes assessing patient health needs, developing nursing care plans, and educating patients on relevant health management information. This position also oversees the operational running of the medical clinical area and patient communication within the electronic health record system.

Main Responsibilities: Clinical Floor Oversight

Prepare clinical area for building opening or closing in accordance with Finger Lakes Community Health hours of operation Redirect nursing team tasks to ensure an efficient patient flow Complete medication reconciliation Ensure the following duties are completed by nursing staff:

Pre-visit planning Inventory restock and ordering Quality Control measures Equipment inspection date checks Sterilization of instruments, equipment, and clinical areas

Teach nursing staff on use of equipment, techniques, and clinical services Perform preventative maintenance on medical equipment

Clinical Services

Assist with minor procedures and exams as outlined in NYS clinical scope of practice Perform patient screenings and assessments as outlined in NYS clinical scope of practice Complete phlebotomy and lab collection and handling as outlined in NYS clinical scope of practice Confirm patient charts have written orders for medications and/or injections from the provider Administer injections and immunizations as directed by ordering provider Review and send medication refill requests to patient's provider Inform patients or designated persons of physician orders, changes in care plan, test results, etc. as outlined in NYS clinical scope of practice

Care Management

Attend morning huddle meeting with patient care team to address patient concerns Review specific populations health care needs and follow-up appropriately Oversee referral process for patients as directed by provider including initiation, follow-up, and documentation Educate patients and family members on health information and medication administration Promote patient's independence by establishing patient care goals Serve as liaison between patient and provider when needed

Medical Record Maintenance

Chart services in patient record including but not limited to:

Referrals, medications, vaccinations, injections, screenings, lab orders/results, patient care goals, patient history, vitals Audit progress notes to ensure all patient history and vital signs have been taken and noted by nursing staff

Chart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and procedures Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures

Education and Qualifications:

A current, unrestricted NYS Registered Nurse License required CPR certification required (can be provided) 1-3 Year of experience working in license The ability to work flexible hours -- some evenings are required A valid government-issued photo identification document is required

Physical Requirements:

Must be able to hear and communicate with clients and staff via phone, web media, and in person. Must be able to lift up to 40 lbs Must be able to sit for extended periods of time Must be able to kneel, crawl, or bend into awkward spaces OSHA Level 1
