Your future role at a glance:

Location: Tulsa, OK

Facility: Ascension St. John Medical Center

Department/Specialty: Total Quality Management

Schedule: FT, Monday-Friday

How you'll make an impact in this role:
  • Drive improved patient outcomes by bridging hospital and outpatient care, ensuring seamless continuity and reducing care fragmentation.
  • Reduce hospital readmission rates by providing ongoing clinical monitoring and early intervention for post-discharge patients.
  • Empower patients through personalized health coaching, education, and support for long-term treatment goals.
  • Coordinate care plans among primary care providers, specialists, and community resources, addressing social determinants of health and medication management.
  • Transform patients into active participants in their recovery, resulting in measurable improvements in quality metrics for the Total Quality Management department.
What minimum qualifications you'll need:

Licensure / Certification / Registration:

  • Licensed Registered Nurse obtained prior to hire date or job transfer date required.
  • Registered Nurse credentialed from the Oklahoma Board of Nursing required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
What additional requirements you'll need:
  • Must be willing to obtain a compact license within 90 days of hire
  • Prior Care Manager, Case Manager in Home Health/Hospice, or Patient Navigator in Ambulatory setting
  • Willing to travel within 40 mile radius of office for in-field component conducting patient home visits and attending specialty appointments (30%)
