Clay County Public Health is now accepting applications to be considered for one open position as a Full-Time Registered Nurse OR Full-Time Social Worker.

Status: Exempt | Monday -Friday 8:00 A.M.-4:30 P.M.

Hiring Salary Range: RN/BSN:  $35.08-$37.72 per hour – Full Salary for position - $35.08-$49.60 per hour

RN-AD OR Social Worker:  $33.36-$35.86 per hour – Full Salary for position -$33.36-$47.06

MINIMUM REQUIREMENTS

  • Bachelor’s degree in nursing or social work preferred
  • Licensed in Minnesota as a registered nurse OR social work license preferred
  • 1-3 years of related work experience
  • Valid driver’s license
  • Ability to pass a background check

DUTIES AND ESSENTIAL FUNCTIONS

The listed examples may not include all the duties performed by all positions in this class.

  • Provide care coordination to individuals eligible for Special Needs Basic Care (SNBC) through UCare; includes assisting members with establishing in-home services, coordinating medical appointments and transportation, helping members navigate complex health systems, assessing for additional resources to meet the person’s needs.
  • Provide all required documentation by preparing and maintaining all client records, documenting service and contact activities according to Federal, State Statutes/Rules as well as County policies/guidelines to allow for quality assurance review and maximization of funding.
  • Other duties as assigned.
