This rewarding position allows you to utilize your expertise in homecare, providing essential nursing support to patients in their homes. As a Per Diem Registered Nurse ONCALL at Stratford Visiting Nurse Association, you will play a crucial role in triaging patient calls that come into the agency via our after-hours call service. Your expertise will be vital in answering questions from patients and caregivers, assessing whether a PRN home visit is necessary to provide essential in-person assessment and treatment. Utilizing your experience as a critical thinker, you'll be responsible for promptly reporting any changes in the patient's condition to their healthcare provider, ensuring continuity of care.

Potential applicants will need at least one year of RN experience and a valid State of Connecticut RN license. Strong organizational and communication skills, knowledge of disease management and have an aptness for teaching patients how to manage their illnesses. The ability to develop a good working rapport with patients, develop trust and work well in an interdisciplinary team are crucial in this role. Applicants must be able to present themselves in a professional manner and have an interest in community health. Home healthcare expereince is preferred and knowledge of the Matrixcare EMR is a plus.
