The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need, including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team-based care in an integrated care setting. We are looking for a mission-driven service leader who works well in a team, has experience with managing and supporting staff, champions quality improvement, leads change, and is excited to innovate. This is a union-eligible position. This is an On-Call position; shift hours and days vary as needed.

On-Call Registered Nurse (RN) Responsibilities
The On-Call RN works on the Patient Care team to provide nursing support, including:
• Providing patient care, understanding orders, health education, and advanced patient care support. • Ensuring primary care team support of patient encounters, medical case management, and quality improvement. • Providing complex care management for patients with multiple chronic medical illnesses and behavioral health concerns. • Supporting patient care teams to ensure efficient clinical flow. • Coordinating care with other care teams and specialists.
KEY RESPONSIBILITIES
Training Responsibilities
• Participates in training medical assistants, nursing students, and nursing volunteers to improve the skills of back-office staff. • Orients new nurse volunteers and clinical and support staff to appropriate policies, procedures, and protocols of the medical clinic. • Supports staff training in quality improvement functions related to their specific position, e.g., basic customer service and patient interaction skills, clinical documentation processes, incident reporting, confidentiality, and privacy. • Provides training on clinical and quality improvement issues as needed and appropriate.
Nursing Responsibilities
• Manages same-day patients with the primary care team. • Provides advice to primary care team walk-in patients and patients calling into the clinic. • Provides nursing support to the patient care team and manages clinic flow. • Provides appropriate patient care visits and manages patient panel under standing orders. • Manages prescription refills and medication modification for patients under standing orders. • Provides complex care management for patients with multiple chronic medical illnesses and behavioral health concerns for the patient panel. • Works closely with staff collecting and reporting contractual data as necessary. • Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency.
QUALIFICATIONS
Education • ASN, BSN, MSN, or equivalent degree. • Graduate of an accredited School of Nursing.
Certification and Experience • Active California RN license. • First Aid & CRP certification. • Experience with electronic medical records preferred. • Experience in managing volunteers preferred.
