This role includes supportive therapy, therapeutic interventions, nursing assessments, and other essential nursing services. As an active member of the treatment team, the Nurse Care Manager ensures individualized, patient-centered care and maintains clinical records in eClinicalWorks.\n \nEssential Duties & Responsibilities:\n\n Build trusting relationships with patients through high-touch engagement.\n \n\n Perform medical triage in collaboration with healthcare staff.\n \n\n Accept and process referrals for the medical respite program.\n \n\n Educate patients and hospital staff about the Medical Respite program and participation expectations.\n \n\n Conduct medication reconciliation and provide medication education in partnership with providers and pharmacy staff.\n \n\n Develop and facilitate chronic disease management and healthy lifestyle programs using evidence-based practices.\n \n\n Coordinate internal and external care, including specialty care, tobacco cessation, housing, transportation, and health benefits.\n \n\n Identify and address barriers to improved health outcomes for medically complex patients.\n \n\n Implement systematic strategies for assessments, enrollment, care planning, documentation, and value measurement for patient care.\n \n\n Participate in a multidisciplinary team to integrate primary and behavioral healthcare services.\n \n\n Offer ongoing consultation with treatment teams, hospitals, specialty providers, pharmacies, insurance companies, patients, and families.\n \n\n Provide crisis intervention as needed.\n \n\n Administer medication injections as required.\n \n\n Assist patients with daily living skills and activities.\n \n\n Support patients in preparing for medical appointments.\n \n\n Encourage and facilitate patient engagement in their community.\n \n\n Enhance programming by networking and collaborating with internal and external partners.\n \n\n \nSCHEDULE: (non-exempt/salaried position)\n\n Monday through Friday 8a.m.-4p.m. OR 9 a.m. \u2013 5 p.m.\n \n\n \nGREAT BENEFIT PACKAGE: (Benefits start Day 1 of employment)\n\n 403B with employer match\n \n\n Paid Time Off (PTO)\n \n\n Medical, Dental, Vision\n \n\n Life Insurance\n \n\n Paid Holiday Days\n \n\n \nPlus more\u2026\n \nABOUT PITTSBURGH MERCY: We're a community-based health and human services organization using person-centered care to treat our area's most vulnerable populations.
