Position Overview:
We are seeking an experienced Registered Nurse MDS Coordinator to join our team at a skilled nursing facility in New York, NY. In this role, you'll conduct comprehensive patient assessments, ensure regulatory compliance, and coordinate interdisciplinary care plans to optimize resident outcomes in a long-term care setting.

Qualifications:
  • Associate's or Bachelor's degree in Nursing (ADN/BSN).
  • Active New York RN license.
  • MDS Assessor Certification (required).
  • 1–2 years in long-term care with MDS coordination experience.
  • Attention to detail, strong communication, and the ability to lead interdisciplinary teams.

Key Responsibilities:
  • Manage the intake and evaluation process for patients in long-term care.
  • Complete up-to-date patient assessments and oversee the interdisciplinary assessment for all nursing home residents.
  • Monitor patient care by assessing procedures.
  • Interview patients and record accurate medical codes.
  • Enter medical codes and support medical billing processes, maintaining accurate medical records.
