Active Home Health, Hospice & Personal Care is looking for a Part-Time and PRN Hospice RN to care for our patients in the Weber-Davis County! Job Requirements:
- Visit and assess patients to complete an initial comprehensive Assessment and ongoing Assessments through end of care.
- Chart all clinical documentation in our EHR software.
- Create and manage patient plan of care.
- Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
- Case manage the patient episode or benefit period, ensuring that all cares are performed and documented.
- Provides medication education and management per physician order.
- Works hand-in-hand with the IDG to ensure patient goals are met and appropriate outcomes are achieved.
- Shares on-call duties.
- Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams.
Job Qualifications:
- Registered nurse with current license to practice professional nursing in the state of Utah.
- Polite and compassionate.
- Excellent observation, problem solving, verbal and written communications.
- Maintains a current CPR certification.
- 1 year Home Health or Hospice experience preferred.
- Must be a licensed driver with an automobile that is insured, registered and in good working order.
- Good communication and smartphone skills.
- Ability to pass a background check.
