Full-time RN position to provide individualized nursing care at the place of residence/home of clients with acute, chronic and complex conditions.
- Home Care Registered Nurse (RN) - MercyOne/Trinity Health Home Care, Mason City
Monday - Friday 8am-4:30 pm; Weekend rotation based on how many nurses are employed. On call only Saturday and Sunday 8am-4:30 pm on your scheduled weekend. No weekday or weekend evening call.

MercyOne/Trinity Health Home Care in Mason City is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated home care team. In this rewarding role, you'll deliver high-quality, personalized care to patients in the comfort of their homes, helping them maintain independence and improve their quality of life.
What You'll Do
- Perform comprehensive patient assessments and identify care needs.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Administer medications and treatments, monitor patient responses, and adjust care as needed.
- Educate patients and families on disease management, medications, and wellness strategies.
- Monitor vital signs and manage chronic conditions with timely interventions.
- Accurately document all care activities and maintain compliance with healthcare regulations.
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Provide emotional support and guidance to patients and their families.
- Participate in ongoing education and quality improvement initiatives.
What You'll Need
- Minimum Qualifications:
- Graduate of an approved Nursing education program, BSN preferred.
- Current Registered Nurse licensure in the state of practice.
- Must have a minimum of one (1) year professional nursing experience.
- Home healthcare experience preferred.
- Strong computer and technology skills required.
- Must have current Driver's license and reliable transportation.
- Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere
to the Compliance Program.
