We are seeking an experienced Travel Registered Nurse to join our Home Health/Hospice team. This role provides compassionate, high‑quality care to patients in their homes while meeting productivity and documentation expectations. The ideal candidate is self‑directed, organized, and comfortable managing a field‑based caseload.
Deliver skilled nursing care to home health and hospice patients across East Baton Rouge and Livingston Parish.
Complete comprehensive assessments, develop care plans, and provide ongoing patient education.
Document all visits and interventions accurately within HCHB (Homecare Homebase).
Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
Meet productivity expectations of 30 points per week.
Participate in required on‑call and weekend rotations.
Travel between patient homes; mileage reimbursed from first to last patient.
Active RN license (state‑appropriate).
Minimum of one year of Home Health experience.
Proficiency with HCHB (Homecare Homebase).
Strong clinical assessment and critical‑thinking skills.
Valid driver’s license and reliable transportation.
Previous hospice experience.
Strong communication and time‑management abilities.
Ability to work independently in a fast‑paced field environment.
Dress Code: Royal blue, black, or navy scrubs.
Mileage: Reimbursed between visits (first patient to last).
