Position Overview:

Seeking an experienced Home Health Registered Nurse to provide high-quality, patient-centered care in the home setting. This role focuses on conducting skilled nursing visits and completing OASIS assessments while collaborating with an interdisciplinary team to support optimal patient outcomes across designated coverage areas.

Schedule & Work Details:

  • Location: Coverage across Lorain, Cuyahoga, and Lake counties
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 40 hours scheduled, 36 hours guaranteed
  • On-Call/Weekend: One weeknight call rotation per month and one weekend per month
  • Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Perform skilled nursing visits in the home health setting
  • Complete accurate and timely OASIS assessments
  • Develop, implement, and update individualized care plans
  • Monitor patient conditions and report changes to the care team and physician
  • Educate patients and families on health management, medications, and treatment plans
  • Maintain thorough and compliant clinical documentation
  • Collaborate with interdisciplinary team members to ensure continuity of care
  • Follow agency policies, procedures, and regulatory requirements

Required Qualifications:

  • Active and unrestricted Registered Nurse license
  • Associate degree in Nursing
  • Minimum of one year of Home Health experience
  • Strong clinical assessment and critical thinking skills
  • Ability to work independently and manage time effectively
  • Valid driver's license and reliable transportation

Preferred Qualifications:

  • Experience with Homecare Homebase (HCHB)
  • Strong communication and patient education skills
  • Familiarity with OASIS documentation and home health regulations
