Job Title: Registered Nurse (RN) – Home Health
Location: South Dennis, MA 02660
Duration: 13 Weeks
Schedule: Day Shift, Monday – Friday
Compensation
- Local Candidates: $1,500 – $1,800 per week
- Travel Candidates: $2,000 – $2,300 per week
Key Responsibilities
- Maintain annual HHRG values at or above national standards as appropriate
- Utilize HHRG values to determine visit frequency and appropriate interdisciplinary care utilization
- Ensure annual patient outcomes meet national benchmarks
- Refer patients to community resources as needed
- Assess patient clinical needs and develop, implement, and revise individualized plans of care
- Address all identified problems within the care plan and ensure accurate documentation
License Requirements
- Active Registered Nurse (RN) license in Massachusetts
- Current CPR certification
Education & Experience
- Minimum of one (1) year of nursing experience required, or six (6) months with a BSN
- Home health and IV therapy experience required
- Bachelor of Science in Nursing (BSN) preferred
- Successful completion of VNA pre-hire medication test
